The United States inappropriately combines and confuses clinical data, that is what is happening medically with a patient, with billing data, under the guise of wanting more information.

It is wasting money. It is wasting time. It is wasting resources. And, sadly, it is wasting the expertise of physicians, leading to further disgust on the part of physicians, many leaving practice.
