I am just struggling to understand why Appalachia. Why not Georgia? Why not Alabama? Why is it the Appalachia area is singled out for such high drug use, high painkiller use.

I have never heard President Biden say that, and I can't imagine that anybody would urge folks, desperate folks, to send their children at great, great personal risk to the United States unaccompanied.
