If the attending physician certifies that there is no appropriate specialist practicing under the insurance plan, the plan must ensure that coverage is provided outside the plan for a second opinion by a qualified specialist selected by the attending physician at no additional cost to the patient beyond that which the patient would have paid if the specialist were participating in the plan.”
